New Patient Appointments
- All new patients must complete the New Patient Packet, as well as send in a deposit, in order to secure an appointment. Please call our office to have the New Patient Packet sent to you.
- Prior to your appointment, we will need to receive your pertinent medical records.
- Please arrive 15 minutes early for your appointment and bring your photo ID, insurance card, records, and a list of current medications. Please also be prepared to pay by cash or check for any fees and/or additional charges.

How to be Referred
We accept referrals from any of the following
individuals or agencies:
Physicians
Patients
Case Managers
Rehabilitation Professionals
Lawyers
Worker’s Compensation
Advocacy or Disability Agencies
We prefer to receive a referral letter and relevant injury related
medical records prior to seeing any new patient.
Payment
Our providers do not participate with any insurance companies and payment is due at each appointment. We are happy to assist you with submitting claims for any out of network benefits you may have.
